Acids and bases formed by ionic bonds, like HCl or NaOH are present as ions in the solutions. So they do contain H+ and OH- ions.
On the other hand acids/bases formed by covalent bonds are not present as ions. They are present as covalent compounds. (For example, organic acids)
What makes it an acid or base can be explained by different acid/base theories. According to popular theories, something which gives H+ ions on dissolution is called an acid and one which gives OH- ions is a base. What differentiates them both are their properties and these theories are just to explain them.
