Bleeding from the mouth just before or during death is a common movie trope. It is like a lot of tropes not really medically accurate. Its just there as a marker for the audience to know who is going to die soon and it makes for a shocking visual spectacle
You bleed from the mouth if you have a cut in the mouth/tongue/lips or somewhere a little up in the nasal area. If you have bleeding in the stomach like a stab wound or a bleeding stomach ulcer you may not always vomit up blood. If the blood is in the stomach for a long time you vomit up stomach contents with dark red almost black blood that is clotted, not fresh red blood. This is called coffee ground vomitus since it looks kinda like coffee grounds.
After death, well its common for gases to build up in the abdominal cavity as the insides are eaten by bacteria. Too much pressure and the contents of the stomach can in some cases be pushed out the mouth. After death the blood does coagulate but in some cases, the non coagulated part which is the blood plasma can have a reddish color and this can be mistaken for blood. Or the blood may re liquify.
