does oxidation only occur with addition of oxygen or can it occur with the addition of any oxygen containing substance (OH for example).

It occurs only with the addition of oxygen, but if any oxygen containing substance is broken down in the equation, oxygen can be added. It also occurs when an atom, molecule, or ion loses one or more electrons in a chemical reaction. When this occurs, the oxidation count of the substance increases, so it doesn't necessarily involve oxygen.
