No. Transfer of heat does not take place in all situations.
Before telling you what conditions are needed to transfer heat, you need to know how heat travels.

Heat travels in 3 ways:
1. Conduction
2. Convection
3. Radiation

Factors needed for heat transfer:
1. The factor that is necessary for the flow of heat from one object to another is that the two objects must have different temperatures. Heat always "flows," or moves, from the hotter object to the cooler one, unless there is some sort of external agent doing work.

2. Factors needed for heat to travel through conduction:
The two objects must be touching each other. This is taking in consideration that only conduction is taking place. Radiation or Convection are not.

3.  There are no factors needed for heat to travel through convection or radiation. The reason for convection being, that convection mostly takes place in fluids which are already woven together with atoms.

4. Radiation also needs no medium for heat transfer. Radiation is the mode of heat transfer through which the sun supplies heat to the earth.
