It is Usually self-diagnosable.
Short sightedness is called myopia.

✨Long Sightedness-
A condition in which objects located at a far distance can be seen, but close ones cannot.
Long sightedness is called hypermetropia.

Short sightedness:A person suffering from this defect can see distant objects clearly but cannot see nearby objects clearly.In this defect,the near point lies farther away than 25 cm.The image of nearby object is focussed beyond retina.
2.It can be corrected by using convex lens.

Long sightedness:A person suffering from myopia or short sightedness can see nearby objects clearly but cannot see the far away objects clearly.The image of distant objects is focussed before the retina
2.It can be corrected by using concave lens.
