Ans. He asked me what my name was.
2. She said to me, ''Are you ready to go out with me?"
Ams. She asked me if I was ready to go out with her.
For model auxiliaries, just use ask / asked /inquire of / inquired of etc as per tense and then put model auxiliaries who / what / when etc and then weite the speech in indirect form as I have shown in examples.
For simple tense problems, use if / whether following asked / inquired of etc.
