In plants, water and minerals are transported through the vascular xylem tissue. Factors responsible for this are:

1) Root Pressure: Water and minerals from the soil enter through the thin cell walls of root hairs, due to osmosis. It helps is the transportation of water and minerals from the soil to the base of the stem.

2) Capillary Action: Water and Minerals rise up to some height in the stem due to Capillary Action.

3) Adhesion: Water molecules make a continuous column in the xylem tissue because of forces of Adhesion and Cohesion among the molecules.

4) Transpiration Pull: Loss of water in plants in the form of water vapour through stomata creates a vacuum which generates a suction called transpiration pull. It sucks the water column from xylem tubes and thus water and minerals are able to rise to great heights even in the tallest plants.

I hope this Helps :)
