The transudates result from leakage of pre-capillary arterioles and accumulation of fluid in the deep retina. In contrast to the retinal hemorrhages, infarcts and exudation which are common to multiple retinopathies, focal intraretinal periarteriolar transudates are specific to malignant hypertension!
