Given,

d = 4 \\ \\ a_{38} = 141 \\ i.e. \: a + 37d = 141 \\ a + 37(4) = 141 \: \: \: \: \: \: \: (d = 4 \: given) \\ a + 148 = 141 \\ a = 141 - 148 \\ a = - 7

To find out the 5th term,

a + 4d = a_{5} \\ \\ -7 + 4(4) = a_{5} \\ \\ -7 + 16 = a_{5} \\ \\ a_{5} = 9

If any doubt, please ask ;)
