Respected Madam:

I wish to state that I lost my identity card in the school premises last week. I tried to search it; I wrote a notice for the same hoping I would get it back but to no avail. I am facing a lot of inconvenience on account of it. Kindly issue me a duplicate identity card. I shall be grateful. Thanking you.
