Honorable Principal Sir,

I, Jannat, a student of class 10, studying in your school, like to state that I am suffering from a serious fever. The doctors in our locality are unable to find out what actually the disease is and what is the cause for it. So local doctors has recommended me to go to Mumbai to a reputed hospital.

Therefore, I request you to please grant me leave for 5 days, that is from, 2nd December to 7th December. I shall surely cope up with the missed classes and assignments. Looking forward for your positive reply.
