and his drill it do whine,
in these molars of mine.

his refers to the doctor who is going to drill in molars of poet as there is cavity in poet's molars

'His' refers to the doctor who was treating the poet. He had cavity in his teeth and so the doctor had come to check and who was drilling the molars of the poet.....
