The biggest difference is particle size. Of all types of soil, sand has the largest particle size, and clay has the smallest. One grain of sand in hundreds of times the size of a clay particle. Because of this difference, sand drains water fast, and clay retains water very well (too well usually).

Also, the particle size difference leads to other differences such as phosphorus content, pH differences, etc. This is not a rule, but on average, clay soils have a bit higher pH, usually around 7–7.6. Sandy soils are more acidic, usually around 5.5–6.7. The lower pH in sandy soil makes some things dissolve in water much easier, such as phosphates, which explains why sandy soils (usually) have low phosphorus content.

The larger particle size allows free drainage, so rainwater, which is slightly acidic due to the carbonic acid, runs through it and flushes other stuff down deeper.

To get soil with drainage that is somewhere in between, mixing clay with sand is not a great idea. Depending on the plant, but soil with medium particle size can hold water, but not too much. Medium particle size can drain, but not too much.
