The answer is:

‘His leg was hurt in an accident (by him).’

Why this is the answer?

In the sentence- “He hurt his leg in an accident”, here the object is “his leg” and as per the grammar rules for changing the voice to passive from active,

the object(his leg) comes to the place of the subject(he) and the subject(he) goes to the place of object(his leg).

There sits the 3rd form of the verb(hurt) i.e. also “hurt”.

There comes a be-verb before the verb(3rd form) i.e- “was”

As the “his leg” is a singular form and the tense of the sentence is past, here the be-verb is- “was”

When the subject(he) is converted to the object(him) it changes itself. Example: I- me, We- us , They- them, He- him etc and there sits a “by” before the object(the converted subject). These are the examples-

As the work(accident) is occurred by himself, it is not necessary to add the “by him” at the last part of the sentence while converting it into the passive voice. So I added it into a bracket.
