1. For simple present tense, it will ‘is/am/are'.

2. For simple past tense, it will be ‘was/were'.

3. For simple future, it will be ‘be'

4. For continuous tenses, it will be 'being'

5. For perfect tenses, it will be 'been'

Then change the main verb into its third form (past participle form).

Use a preposition such as ‘by’ and bring the subject after it changing its form into objective case e.g. they - them, I - me
