So in a) the force is being applied on the person through a finger which has a large are of contact. The pressure from the force applied is evenly distributed throughout the area of contact

In b) the same amount of force is being applied through a syringe. We know that a syringe needle has a pointy tip so the area of contact between needle and skin is very less. Since the same force is applied on lesser area there is more pressure and all the force is concentrated to that one point.
