the major difference between coming together and holding together is that in coming together there is personal constitution of each state while and holding together there is no any personal constitution

In coming together federation :
The states (countries) come together on their own will and consent.
They can at any time quit the federation and become and independent nation.(but this doesn't happen coz the purpose is mainly security by forming a large unit )
The name of the states is not mentioned in the Constitution .
The participating nations can have their own separate constitutions
All the participating states have equal rights.
Eg . USA
In Holding Together Federation
The states are bound together by a national or central power (Central or Union Government as it is in India).
No state can on its own leave this federation to form an independent nation.
Participant States can not introduce their own constitution.
The names of the states are mentioned in the Constitution
Some provinces. may or may not have equal rights.(Like in India Jammu and Kashmir enjoys special status due to Article 370)
Eg as discussed India.
