Hydrogen and Helium were the first elements to form in the universe. All other elements are created by nuclear fusion inside stars. When some stars explode as supernova, these elements are spread throughout the universe.

The solar system formed from dust clouds 4.5 billion years ago.

Basically clumps of rock came together to form the earth.

There are also some theories that another planet hit earth, and a part of the earth got separated and this has become the moon.

There is also a theory that the earth might have initially been a super-earth, that is maybe around 10 times the mass of earth. This super-earth might have been hit by another planet of similar size. After both planets disintegrated, the present earth formed. In this case, the earth would be a second generation planet.
