Hydroxyaptite or $Ca_5(PO_4)_3OH$  is that insoluble substance which constitutes the tooth enamel. There is an equilibrium set up in mouth by which the remineralization (formation ) and  demineralization(dissolving) of the tooth takes place continuously for a healthy tooth as well.

 $Ca_{5}(PO_4)_{3}OH(s){(demineralisation)}\rightleftharpoons{(Remineralization)} 5Ca^{2+}(aq) +3PO_{4}^{3-}(aq)+ OH^{-}(aq)$

But if sugar is absorbed by teeth and fermentation occurs and $H^+$ is produced that tries to combine with $OH^-$ and also $PO_4^{3-}$ forms $HPO_4^{2-}$ and equilibrium is disturbed. And the process continues resulting in tooth decay.

Fluoride replaces the $OH^-$ in hydroxyaptite and thus prevents tooth decay.
