Before the beginning of the 20th century, the only means of obtaining pearls was by manually gathering very large numbers of pearl oysters or mussels from the ocean floor or lake or river bottom. The bivalves were then brought to the surface, opened, and the tissues searched.

Answer
Natural pearls were gathered by pearl divers that would manually search the beds of oceans, lakes and rivers, perhaps finding only 3 or 4 quality pearls for every ton of oysters brought to the surface. ... Divers would suffer from decompression sickness, the cold waters and deep-sea pressure.
