Chronic Diseases are those diseases which an individual can control with treatment but this cannot be cured by medications.
E. g. Diabetes, Asthma, Cancer etc.

Poor health is a condition when an individual is not mentally or physically unfit for carrying out their daily jobs. This is due to poor nutrition.
