y+4=-13(x-12)

=> 13(x-12)+y+4=0

=> 13x-156+y+4=0

=> 13x+y-152=0

=> 13x+1y+(-152)=0 Compare

this with ax+by+c=0,we get

a=13,b=1,c= -152

Therefore,

Standard form:

13x+1y-152=0
