Gaseous exchange takes place in blood level as blood flows through viens and viens and so thin that gasses can pass through it. And in real it does happen.

The cells that need oxygen contain low amounts of oxygen and the blood contains a high amount of oxygen so a diffusion gradient is set up and oxygen moves into the cells from the blood ... the same is with carbon dioxide as it moves out of the cells into the blood as it is produced by the cells during respiration ...
