A prejudiced person may not act on their attitude. Therefore, someone can be prejudiced towards a certain group but not discriminate against them. Also, prejudice includes all three components of an attitude (affective, behavioral and cognitive), discrimination just involves behavior.

Prejudice leads to discrimination because if one do partiality with a community of specific gender, religion, color or because of any other reason it means that the person is discriminating against that particular group.
