The forces of cohesion and adhesion cause the water molecules to form a column in the xylem. Water moves from the xylem into the mesophyll cells, evaporates from their surfaces and leaves the plant by diffusion through the stomata.This is known as transpiration

In plants all parts contain water especially the water is found near the stomata of leaves and from there a channel filled with water is connected till the roots ...so due to the forces of cohesion and adhesion the water molecules form a column in the xylem. Water moves from the xylem into the mesophyll cells, evaporates from their surfaces and leaves the plant by diffusion through the stomata.
