How is ozone formed in the upper atmosphere? What causes its damage?

Ozone is formed by the chemical reaction of oxygen with sun energy etc. this all lead to the formation of a oxygen blanket in the stathosphere. It is damaged by the green house gases, like carbon dioxide, carbon monoxide, CFC's etc.
