This requires alkalinization of the medullary interstitium. In a patient with hypokalemia, the H+/K+-ATPase activity in the MCD is increased. The net effect of this H+ ion secretion into the lumen of the MCD is the addition of K+ and  ions to the interstitial compartment (Figure 4-9). As a result, the pH in this compartment rises, converting  to  anions, which precipitates with ionized calcium. The rise in pH, however, is too small to raise the concentration of  appreciably. Hence, precipitation of calcium carbonate may provide a nidus for the precipitation of calcium phosphate.
