It is determined by what happens at the time of fertilisation.If a sperm carrying X chromosome fertilises with the ovum carrying X chromosome then the baby would be a girl and if the sperm carrying Y chromosome fertilises with an ovum carrying X chromosome then the baby would be a boy.

In the old days when echographic pics were not so good they would look b/w the legs of the foetus seeing a stick and two round balls at the base meant a boy seeing a groove would mean a girl of course sometime they could not tell not being able to get an unimpeded view of gential area.
