See, whatever is asked in the question you have to do it accordingly, in formal letters, u do not need to write much and in informal, it is a flow of conversation where ask about each other's well being and then discuss the original fact. In formal letter, just learn this sentence'with due respect and humble regret, I beg to state a few lines before your consideration' then give your introduction: who u are, from where do you belong, what is your problem, etc.
In the last paragraph, end up saying,'kindly grant the permission/kindly take necessary action. Hoping for a positive result'
