Just write your complaint on a plain paper and add as much information as u can and attach other relevant other documents, pertaining to the person against whom the complaint is being filed. You may/may not mention your name. Give one copy with the enclosures to the local income tax office, and address a copy to the nearest Pr.CIT. Whether the assesses will be nabbed depends on the findings of the officer and the satisfaction/reasons accorded by the commissioner who ascertains whether the information is concrete/ worth taking action or not .
