In order to get Christian minority certificate candidate has to fill up a form declaring his/her name, father's name, his/her religion along with a self declaration for family income.

The other required things are a cancelled cheque as well as an affidavit stating that he/she is not getting any financial assisstance from any other sources. An alternate to the affidavit is either a community certificate from principal of school or college or religious authority.
