In formal letter, you have to write the address in first line and after leaving one lineu have to write the place and the address that grom where u r sending the letter. After that, u will write subject and date after leaving one one line. This is the starting of formal letter. Ending will be then when u will write YOURS SINCERELY and your NAME.
