How to write a letter to bank manager to handover my atm card to my father?

Your letter should look something like this:


TO: (Name of Bank Manager) Bank Manager (Name of Bank) (Bank Address)

RE: Issuance of ATM Card to Mr/Ms (Name of your friend)

Dear Sir/Ma`am,
This is to authorize my father , Mr./Ms. (name of father ) to claim my issued ATM Card on my behalf as I am currently out of town (or whatever your reason is) and does not have the time and ability to drop by your bank anytime soon.
My account number is __________, and with (friend`s name) is my claiming stub (if any), my ___ ID (preferably government-issued), (and state other IDs you have given your friend) for my account verification purposes. If there are any other forms of formalities that you require, please let me know.
You can contact me at _______(your contact number) for verification and more information regarding this. Any courtesy extended to him/her on my behalf with be very much appreciated. Thank you.

   Sub: Request to hand over my ATM card to my father.

I, Xavier from Kottayam had applied for an ATM card on 5/1/2022. I got a notification from the bank that the ATM card will be delivered on 12/1/2022. I won't be able to collect the card as I will be leaving from Kottayam to  Trivandrum for an interview purpose and therefore won't be able to collect my card and I request you to hand over it to my father Mr. Joseph. I have attached his Aadhar number. I kindly request you to accept this letter and oblige.
