There are some situations wherein a customer can lose his pass book - during transit, shifting of house to some other place etc.,

In such case, the customer has to submit a written representation to the bank branch and the bank will provide a duplicate pass book.

The customer can also demand the period from which the details are required.

The bank collects service charges for issue of such duplicate pass book from the customer.

You can use the following format for getting a duplicate pass book
