Dear Sir,

Please be informed that earlier today in the afternoon, I had accidentallyfallen from the bike while going home from office. Due to which I have met several minor injuries. I have severe body aches and swelling in my leg. Walking and moving around aggravates my pain. I request you to kindly grant me leave for three days i.e. from 18th August, 2015 to 20thAugust, 2015 so that I can take rest and recover my health as soon as possible.
