Write your address, date on left hand side and on the right side write, ' to the principal' below it write the respective School's name. Leave one life and write the subject. Remember the subject should end on one line (it should be short but conclusive about your problem) Then give a small intro in 1st paragraph and 2nd Paragraph should be the main body, your problem and 3rd paragraph should be thanking and hoping for your earliest reply.
