Gender identity is most likely based on regions of the brain or may be the overall gender balance of the brain, depending on the availability and exposure of different types of hormones during gestation. Most of the time a male fetus gets male hormones and female fetus gets female hormones, but not always possible. This may happen sometime due to rare genetic disorders, though many things yet to be thoroughly understood.

-- In human beings.......when there is a sexual intercourse there are 46 chromosomes shared.......that is the father shares 23 chromosomes ( X, Y).....nD the mother shares 23 chromosomes ( ONLY X ).......so that is why we have 46 chromosomes in general. .........if the father's X chromosome goes nd fertilise with mother's X chromosome.....the baby results as a female. ..... ..If father's Y chromosomes goes nd fertilise with mother's X chromosome......the foetus will be a male.........hence you might have seen in SAAS BAHU SERIALS that If a girl is born ......everyone in the family blames the mother. .......but scientifically the father is responsible for the gender of foetus. ..........so If a foetus have more than or less than 46 chromosomes it results in transgender. ........in the beginning though the transgender acquires male character, but during puberty......he/she .......loves to dress as a girl, gets addicted to makeup .....etc.....
