ANSWER:

The sentence which is formed after the voice change is 'That is not seen by me'.

EXPLANATION:

When the "sentence is in active voice", the subject is performing the action. So the subject must be placed before the action, in the sentence.

Example: I dance well. (Where I is the subject and dance is the action)

When the sentence is written in passive voice, "the subject is being" acted on by the verb. So the subject comes after the 'action in the sentence'.  

Example: Cooking is something I do. (Where cooking is the action and I is the subject)
