Short answer: Study, don't just read.
-----
Reading and studying are two different things. When you study a topic for the first time, you are actually 'reading' it and not studying completely.
Read twice, thrice and study once
The first time or the second time, read it like a novel or a story book and know what is what and what goes where. You know things by this phase.
The next read you give it, close your eyes or book and try to recite the answer in your mind.
Do you see the answer? You will. You will find words floating in your mind ready to be plucked and made into a sentence, but you won't be able to organize it as you didn't study yet.
Look into the book and read it again. Connect those words that you could see with the words in book and, read.
Now repeat again. Close your eyes and try to form a sentence or remember those terms.
This is 'studying'.
When you are looking into the book, your mind will register it as a known point as you have already read it, but the second you close your eyes, you are opening gates to your unconscious mind ( your memory stores ) which doesn't know what is in the book. That is what happens in exams.
So, study, not just read and you will do fine in your exams.
Some tips that will help:
● Write down the points in a short notes
● Make sure to revise the topic as many times as you can.
● Make mnemonics and other clue marks for difficult terms to keep them in memory for long time.

I think there are several reasons for this:-
1. this happens with you because you have a fear of exams even if you're well prepared.I think you have the feeling of fear mainly in maths or science.
2.I think you always memorise the answers and when u sit on the exam chair you forget everything just because of fear.
3.You can have a pressure from your parents or teachers or anyone else to get more marks in especially in maths and science.
You can overcome it by:-
1.Thinking that exams are not going to eat us.No matter how hard the exams are..
2.Not to memorise the answers.Understand them and write them in your own words.
