I have done something; oh, God! I've done something real at last. Why does Andrew say this? What does it mean?(Please make the answer brief and to the point)

Andrew says this because he had been able to keep his promise given to Joe Morgan; he had been able to save Mrs. Morgan as well as his newly born baby after it had been born lifeless.  

It means Andrew felt contented and jubilant at his own efforts and the achievement. In his newly established career he had something to think proudly about. This incident really made him famous in Blaenelly.

Andrew Manson is a doctor whose primary aim is to save lives. When he comes home after midnight and Joe Morgan calls him up for his wife, he was very tired by then but still, he decides to help Joe.

After attending on Joe's wife, Susan Morgan, he helps in not only saving her life but the mother also delivers a baby.

It was very nearly a stillborn baby but somehow Andrew helps in getting the baby get a life.

When Andrew sees this, he is overjoyed and that is why he says those lines.
