In this situation I would ask for the persons for a medicine ( which we normally takes during fever ) if that one is effective the well and good , and if not I will keep a cold wet towel on my friend's forehead, hand etc. so, that he can his body temperature gets cool down for some period of time amd he could feel better .
If all of these doesn't give the good result I would ask the concerned person ( ticket collector or the station master ) to stop the train to the nearest station and my friend to be get admitted in hospital.
