Ask them if they need any help and if their condition does not look good then do the following. Call someone they know like friend or family and tell them about their condition . Make sure to keep the person warm. Record the temperature with your palm and provide them with fever reducer pills or use wet cloth to reduce the temperature. As soon as his/her station arrives help them to get off the train and stay with him or her till the time someone arrives.
