I think the phenomenon that the doctor demonstrate is : language bais when the doctor heard some one talk whiteout proper English, he treated her poorly. But when he faced someone the speaks proper English, he treated them nicely which indicate that he had a bais toward the people that unable to speaks his mother tounge.
