For a perfect balance he should have had 11 heads, one in the center and 5 on each sides. However the 10 heads is a metaphor and not to be taken literally. The ten heads represent numerous views for each action that he had to take and struggle doing so.

Ravana was a scholar, born to a great sage and an asura, he had complete knowledge of the Vedas, complete complete knowledge of all forms of weaponry, a musical genius who played the Sitar, the instincts and failings of an Asura. This constant internal conflict between right and wrong, seen thru his actions, led people to believe that he had multiple heads/personalities.

Similar logic can be applied to lord Bramha, he doesn't literally have 4 heads but is a pictoral representation that he can see or better still be aware of everything that's happening around him.
