the doctor interprets that if johnsy continued with her pessimistic behaviour she would have only one out of ten chances of her survival.
but in order to encourage johnsy and to boost her morale Sue turns the doctor's verdict the other way round that she had ten out of one chances of survival.

its just a similie that a prsn feels happy after walking down the lanes of NY.
