Well it is actually a long and complicating sequence but in can be understood in this manner.

The form that is taken in by plants is Nitrate.

When the nitrogen in the air comes down into the soil it under goes some changes. Since the plant cannot take it as it is it changes into ammonia this process is called ammonification then from ammonia it changes to nitrates this process is called nitrification . Then the plants absorb it.

This is just a summery of the complex process.
