in ur address all the three lines should have comma
in formal in the subject there should be a full stop
yours faithfully and yours affectionately and comma
your name and full stop

You need to put a comma three times in any kind of letter.
1. In between the date, For example:- 23rd january,2013
2. After the salutation. For example:- Dear Priya,
3. At the end. For example:- Your most obedient student,
