With due respect, I want to say that I Raj Sen (Your Name), Student of class vi B (Your Class and section) of your respected school wish to say that, I am suffering from high fever from last 2 days, (state your reason) Yesterday I went to the doctor and she recommends to take rest for 3 days.
Today in 4th class I faint. So, I am requesting you to grant me leave. I want to go home now as I am not feeling well. Also, I notified this at my home and my mother is coming to school to take me home.
