I am not entirely sure what you mean, but I think you are asking how to write a note explaining your absence from class. Be honest! Explain what prevented you from attending class. For example, you might write:

Dear Professor _______,

I am so sorry for missing class yesterday. I had a terrible toothache, and the only appointment I could get with the dentist was during the time I had class. I will be sure to connect with another student and get the notes from class and get any assignments to you in a timely manner. Thank you so much for understanding.
