I, alongwith my wife Jaya, has been holding a joint bank account in your esteemed bank and our account no. 493695712365 for the past 5 years.  We have decided to operate the cash transaction individually.  We request you to help us in converting our Joint Account to Single Account. 

We have attached the Xerox copy of the bank passbook and our Aadhar Card copy for your reference.
